  • Describe the Project, its location and its objectives: It is estimated that almost 30 million people in the United States have diabetes and another 10 million have it undiagnosed and do not realize it. Page County, Virginia seems to mirror these statistics as it is estimated a little over 10% of Page Free Clinic patients have diabetes. This project will partner with the Page Free Clinic in Luray, VA and provide diabetes management and education through various channels: physical activity, medication treatment, and nutrition management. This project will also attempt to identify those patient that are undiagnosed or who are pre-diabetic. The objective of this project will be to help those pre-diabetics get their blood sugar under control before they become full-fledged diabetics and help reduce the hemoglobin A1C (HA1C) of diabetics (a blood test that monitors average blood sugars over a several month period). The program will look at HA1C and have a goal to get this below 7 for patients in the program. Additionally all participants will meet with a dietitian to have their diets assessed. The object will be to educate patients on healthy shopping and eating. The last measurable outcome will be physical activity – looking at weight loss and days per week patients are exercising.

  • Describe how the project will benefit the community and/or improve the lives of the less fortunate: The Luray Rotary Club will partner with the Page Free Clinic; the Page Free Clinic provides free medical care to Page County residents without health insurance who earn between 0-200% of the federal poverty level. It is estimated about 20% of Page County residents do not have health insurance. The Page Free Clinic provides over 700 free medical visits each year. This project will directly benefit lives of the less fortunate by providing diabetes education and medical care to those who normally would not have access. The Page Free Clinic conducted a study and found that 69% of their patients with diabetes had uncontrolled diabetes, meaning the pharmaceutical regimen wasn’t enough. This program will provide patients with education, physical activity and nutrition therapy. These treatments, in conjunction with the pharmaceuticals, should provide a much more robust treatment improving the health and lives of the patients involved with this program.

  • Describe the Rotarian hands-on activities in the project (i.e. non-financial participation): Rotary members will provide volunteer hours in a variety of ways:
    • Data entry – taking information from patients files and putting it in the computer system
    • Provide support to the patients – ensure patients are following their care plan by calling or email patients. If there are patients that need help or are not following their plan, the Rotary member will let the providers at the free clinic know. This is currently a limiting step at the free clinic, as current volunteers and staff are limited.
    • Checking in and checking out patients as they arrive at the free clinic

Additionally the Page Free Clinic will post on their website and facebook that Rotary is a partner and providing financial assistance.
